Appointment Scheduling and Deposits

Legal Entities: UPLIFT NEW YORK, BROOME TATTOO CORP, LES INK CORP - UPLIFT Tattoo & Piercing further

All UPLIFT Tattoo & Piercing artists require at least 48 hours notice to be able to accommodate any changes to your appointment. If you need to reschedule your appointment and inform UPLIFT Tattoo & Piercing about it at least 48 hours in advance, you may make a one-time transfer of your posted deposit towards a rescheduled appointment. If you cancel your appointment in less than 48 hours or do not show up to your appointment, your deposit will be forfeited, and you will be required to make a new deposit in order to schedule a new appointment. ALL Deposits are non-refundable and nontransferable.
A DEPOSIT OF $50 WILL GO TOWARDS PRICE OF THE TATTOO AND $10 IS TAXES AND FEES.
All our artists require at least 48 hours notice to make any changes to your appointment. Giving less than 48 hours notice for a cancellation will result in the forfeit of your deposit. If you cancel or do not show up to your appointment, your deposit will be forfeited. If you need to reschedule your appointment you may transfer your deposit over ONE time, as long as you give more than 48 hours notice. If you cannot make it to your rescheduled appointment your deposit will be forfeited and you will need a new deposit to book again. ALL Deposits are non-refundable and nontransferable.

To have UPLIFT tattoo my body, and in consideration of doing so, I hereby release UPLIFT Tattoo & Piercing, its employees, and its agents from all manner of liabilities, claims, actions and demands in law or in equity, which I, or my heirs, have no or hereafter by reason of my complying with my request to be tattooed. I understand that I will be tattooed using appropriate instruments and techniques to ensure proper healing for my tattoo. I agree to follow the procedures outlined in the UPLIFT Tattoo & Piercing aftercare instructions until healing is complete. I understand that my tattoo takes 1 month or longer to fully heal. I acknowledge that I am at least 18 years old. I acknowledge that all the information above is correct to best of my knowledge.

* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

* I have not traveled internationally within the last 14 days.

* I have not traveled to a highly impacted area within the United States of America in the last 14 days.

* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.

* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.

* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

I hereby release and agree to hold UPLIFT Tattoo & Piercing harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from UPLIFT Tattoo & Piercing. I understand that this release discharges Soho Tattoo & Piercing from any liability or claim that I, my heirs, or any personal representatives may have against the shop with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from UPLIFT Tattoo & Piercing. This liability waiver and release extends to the tattoo shop together with all owners, partners, and employees. We may take pictures, and post them on our social media websites for marketing purposes. We may also post your images on our website.

Before your appointment, please be well rested and eat two hours prior to your appointment.

Face mask must be worn at the premises.

Please refrain from bringing food, pets, and friends with you.

Use of Images and Video

UPLIFT Tattoo & Piercing may take pictures of tattoo and piercing made on your body by UPLIFT Tattoo & Piercing and post them on UPLIFT Tattoo & Piercing social media websites for marketing purposes. Unless you consent to your image being posted on UPLIFT Tattoo & Piercing website, to protect your identity, your face will not be part of the posted images or will be blurred out.

PROMOTION POLICY

This promotional offer is subject to the following restrictions and terms and conditions.

The promotion excludes the following artists: Zay, Val, Yulia, Jakki, and Anastasia. This means that any products, services, or benefits offered in this promo will not be available to these specific artists.

The exclusion of the mentioned artists is final and non-negotiable. No exceptions will be made.

The promo is valid only for eligible participants who are not among the excluded artists mentioned above.

Eligibility for the promo is determined at the sole discretion of the organizers. They reserve the right to verify the eligibility of participants and exclude anyone who does not meet the criteria.

The promo is subject to availability. The organizers may limit the number of participants or the quantity of promotional items or services offered.

The promo cannot be combined with any other offers, discounts, or promotions unless explicitly stated otherwise.

The organizers reserve the right to modify or terminate the promo at any time without prior notice. In such cases, no compensation or alternative benefits will be provided to participants.

Any decision made by the organizers regarding the promo, including but not limited to eligibility, exclusions, and interpretation of the terms and conditions, is final and binding.

Participants must comply with all applicable laws and regulations while participating in the promo.

By participating in the promo, participants agree to release and hold harmless the organizers, their affiliates, and their respective officers, directors, employees, and agents from any and all claims, damages, or liabilities arising out of or related to the promo.

The promo is void where prohibited or restricted by law.

Release

By hiring UPLIFT Tattoo & Piercing to apply a tattoo or/and piercing to your body and agreeing to these Terms and Conditions, you agree to the following release:

Upon hiring UPLIFT Tattoo & Piercing, Les Ink Corp, Uplift New York, Uplift Miami or Broome Tattoo Corporation to render their services to me and accepting these Terms and Conditions, I hereby release and agree to hold UPLIFT Tattoo & Piercing, its owners, partners, employees, and assigns, harmless from, and waive on behalf of myself, my heirs, and representatives, any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may arise in connection with or as a result of any services rendered by UPLIFT Tattoo & Piercing. Specifically, I release and discharge UPLIFT Tattoo & Piercing from any liability or claim that I, my heirs, or representatives may have against UPLIFT Tattoo & Piercing with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services rendered to me by UPLIFT Tattoo & Piercing.

Safety and Security

For your safety and security and safety and security of UPLIFT Tattoo & Piercing personnel, all common areas and artists’ operating pods are equipped with video surveillance cameras. All visitors and patrons of UPLIFT Tattoo & Piercing are being video recorded while on premises of UPLIFT Tattoo & Piercing. All video recordings & costumers personal information are kept confidential and may only be released in response to legal requests (subpoenas) or court orders for legal proceedings.

I agree to follow the recommendations outlined in the UPLIFT Tattoo & Piercing aftercare instructions. I understand that my tattoo/piercing may take at least one month to fully heal. I acknowledge that I am at least 18 years old. I acknowledge that all the information above is correct to best of my knowledge. To have UPLIFT tattoo my body, and in consideration of doing so, I hereby release UPLIFT Tattoo & Piercing, its employees, and its agents from all manner of liabilities, claims, actions and demands in law or in equity, which I, or my heirs, have no or hereafter by reason of my complying with my request to be tattooed. This liability waiver and release extends to the UPLIFT Tattoo & Piercing tattoo shop together with all owners, partners, and employees. We may take pictures, and post them on our social media websites for marketing purposes. By signing form while booking we may also post your images on our website.

Pricing Policy: The cost of your tattoo or piercing service is subject to variation. The final price of your tattoo will depend on various factors including, but not limited to, the design's complexity, size, placement on the body, and the time taken to complete the artwork.

Artist Hourly Rates: Our artists work at different hourly rates, which may influence the final cost of your tattoo. These rates are reflective of each artist's experience, expertise, and demand.

Quotation and Final Price: We do not provide exact quotations for tattoos prior to the completion of the service. While our artists can give an estimated range based on your initial design concept, the final price will only be determined upon the completion of the tattoo. By agreeing to our services, you acknowledge and accept that the final price may be different from any preliminary estimates.

Consultation: Prior to your tattoo session, you may have a consultation with your chosen artist to discuss your design. Please note that this consultation does not guarantee a fixed price for the service.

Payment Obligation: Upon completion of your tattoo, you are obligated to pay the full amount as determined by the final price. Payment must be made in full immediately after the service is rendered.

Changes to Design: Any changes to the initial design or concept, which are made during or after the tattoo session, may incur additional charges. These changes might affect the total time spent and resources used, thereby impacting the final price.

Cancellation Policy: If you need to cancel or reschedule your appointment, please do so at least 48 hours in advance. Failure to provide timely notice may result in a cancellation fee.

Refusal of Service: UPLIFT Tattoo & Piercing reserves the right to refuse service to anyone for any reason. This includes, but is not limited to, refusal based on inappropriate behavior, health conditions, or if the requested tattoo violates our ethical or professional standards.

Age Requirement: Clients must be 18 years or older to receive a tattoo. Valid photo identification is required.

Health and Safety: We adhere to strict health and safety standards to ensure the well-being of our clients and staff. You agree to follow all aftercare instructions provided by your artist.

Liability: UPLIFT Tattoo & Piercing is not liable for any adverse reactions or complications that may arise during or after the tattoo process.

Right to Modify Terms: We reserve the right to change these terms and conditions at any time without prior notice.
